Make the Diagnosis: Is the Clue in the Creatinine?

Presentation

A 56-year-old woman asked her doctor about a change she noticed in her nails over the last few months. They had turned white about halfway up the nail, but the white area didn’t seem to be “growing out” as an injury to a nail normally would. The white line across the nail was present in all 10 nails on her hands. The doctor looked at her most recent blood tests and noted elevated creatinine levels.
